Evidence in one minute

  • Exclude other causes first: venous disease, DVT, lymphoma or pelvic tumour, heart failure and drugs (sirolimus, valproate, tyrosine kinase inhibitors). New adult leg lymphoedema can be the first sign of lymphoma. Rigas 1981 · Martin 2012
  • After gynaecological cancer: leg lymphoedema 1–56% (0–69% after cervical cancer), driven by lymphadenectomy extent, node count, radiotherapy and BMI >25; sentinel mapping is protective. In SA about 30% have a measurable volume difference 1–2 years after pelvic radiotherapy (very low). Decorte 2025 · Bona 2020 · Khutjwe 2020
  • Prevention at pelvic surgery: preserving the circumflex iliac nodes cut 2-year leg lymphoedema (cervical 4.3% v 22.9%) — low; immediate lymphatic reconstruction after iliofemoral lymphadenectomy prevents about 30 per 100, but probably avoid it after limb melanoma or SCC. Wang 2024 · Gloviczki 2026
  • Prophylactic stockings delay rather than prevent leg lymphoedema after groin or pelvic node dissection — low. Gentileschi 2026 · Stuiver 2013
  • Microsurgery: no RCTs; LVA and VLNT reduce volume by 34–47% (very low) and cellulitis (low; revised 2026-10-08). LVA where functional channels are seen on ICG; VLNT for advanced disease at higher morbidity. See the lymphatic surgery topic. Gloviczki 2026
  • Primary lymphoedema surgery (485 patients, 55 series): complications LVA 1%, VLNT 13%, liposuction 11%, excision 46% — very low. Gaxiola-García 2024
  • Exercise is safe and gives small benefits — very low; a Delphi framework gives 26 recommendations. Wittenkamp 2025 · Davies 2026
  • Genetics: a gene panel finds a likely cause in about 22% of primary lymphoedema (FLT4 and FOXC2 commonest); familial disease should prompt genetic testing. Michelini 2016

Secondary leg lymphoedema after cancer treatment

  • Gynaecological cancer: surgical approach is irrelevant; sentinel mapping figures are non-randomised comparisons (JBI 4/6). Dessources 2020
  • Groin dissection for melanoma: leg lymphoedema 59%; fewer, smaller inguinal nodes go with worse lymphoedema; EAGLE FM trial. Campanholi 2011 · Adondakis 2023 · Lee 2024
  • Prevention packages: after radical hysterectomy, self-MLD, 15–30 mmHg hosiery, exercise and education cut 1-year leg lymphoedema from 34.5% to 13.6% (OR 0.30) — low, the curve still rising at 12 months. Early CDT and exercise 15% v 32% — very low. Class 1 stockings with exercise gave identical 12-month incidence (31.9% v 31%) with later onset; class II (23–32 mmHg) for 12 months looked effective in one trial (3.4% v 38.7%) — low. After inguinal dissection, 77% v 84% at 12 months with or without class II stockings; after melanoma groin dissection, compression and self-drainage delayed but did not prevent lymphoedema — very low. Conservative treatment

Veins, arteries and the leg lymphatics

  • Overlap with venous disease: venous hypertension accompanies leg lymphoedema; severe venous disease destroys dermal lymphatics; venous ulcer beds lack lymphatics; deep lymphatics act as collaterals after DVT; skin lymphatic clearance falls in acute DVT. Varicose vein surgery rarely helps the swelling. Kim 1999 · Bollinger 1982 · Eliska 2001 · Tosatti 1978 · Husmann 2006 · Földi 2000
  • Textbook guidance on phlebolymphoedema: venous failure underlies 20–30% of limb lymphoedema and is often missed; IVUS finds iliac stenoses that venography misses, and stenting or reflux treatment can relieve the oedema; CVI was the cause of 72% of secondary lymphoedema in Canadian LIMPRINT data; treat venous disease first, but a normal or fast lymphoscintigram does not exclude a lymphatic component. In young patients, lymphoedema–distichiasis (FOXC2 carriers all have great saphenous reflux), Klippel–Trenaunay and Milroy disease combine venous and lymphatic malformation. Superficial ablation series report improvement, uncontrolled. Neligan 2016 · Gloviczki 2024 · Lee 2018 · Liu 2021 · Tretbar 2008
  • After arterial surgery: swelling after femoro-popliteal bypass reflects raised capillary filtration as well as divided lymphatics. A renal transplant lymphocele causes ipsilateral leg swelling. Stranden 1982 · Lorimer 1975
  • Leg lymph pumping falls with age, most in postmenopausal women. Unno 2011

Primary lymphoedema and genetics

  • Classification: in 1 013 patients, hyperplasia goes with FOXC2 and GATA2, initial aplasia with FLT4; proximal obstruction causes distal "die-back". No FOXC2 or FLT4 mutation is found in about 75% of familial cases; lymphoedema-distichiasis arises from FOXC2 rearrangements as well as point mutations. Liu 2023 · Fyfe 1982 · Michelini 2012 · Fauret 2010
  • Commercial-laboratory variant series (MAGI; authors employed by a testing laboratory) on transcription factors, RAS-MAPK and HGF/MET — interpret with caution. Gelanová 2025 · Belanová 2025 · Kozáčiková 2024
  • Turner syndrome: lymphoedema persists in most; lymphoscintigraphy is abnormal in 15 of 18. Rothbauer 2015 · Welsh 2006 · Bellini 2009
  • Children: UK childhood audit; lymphoscintigraphy in congenital dysplasia; drug-induced oedema mimics lymphoedema (valproate); osteopathy is promoted on adult and laboratory data only. A family account describes congenital leg lymphoedema worsened by infant Z-plasty. Todd 2014 · Bellini 2014 · Singh-Grewal 2016 · Jackson 2025 · Ziegler 2012
  • Genital lymphoedema can signal Crohn's disease or tyrosine kinase inhibitors. Reitsma 2012 · Mathivet 2025

Gaps and warnings

  • Few trials of conservative therapy are specific to leg lymphoedema; most prevention trials are small and conflicting.
  • No SA data on lymphoedema after melanoma groin dissection.

For the therapist

  • Do: look for venous disease, heart failure and drug causes before labelling leg swelling lymphoedema; use compression and exercise (cycling and interval training are safe); refer young patients with a family history for genetic assessment.
  • Stop: expecting short-term prophylactic stockings after groin or pelvic surgery to prevent lymphoedema — they mainly delay it (low).
  • Watch for: genital swelling with leg pumps; new adult leg lymphoedema without a cause (exclude lymphoma or pelvic tumour).
  • Certainty: conservative leg-specific trials are few; most statements low to very low.

For a vascular surgeon in South Africa

  • SA case reports (1959–2026): neonatal leg and scalp oedema; generalised primary lymphoedema with pleural effusions; arm and face lymphoedema with lymph pericardial effusion; infant leg lymphoedema confirmed on repeat lymphoscintigraphy. Look for effusions in generalised or upper-body primary lymphoedema. Griffiths 1959 · Warman 1979 · Househam 1982 · Nel 2026
  • Tuberculous lymphadenitis (a quarter of Durban lymphograms, 1975) is a local cause of secondary leg lymphoedema. Grant 1975
  • Discuss circumflex iliac node preservation with gynaecological oncologists, and expect leg swelling after femoro-popliteal bypass to have a filtration component.
