Evidence in one minute

  • Lymphatic filariasis is the leading cause of lymphoedema worldwide (>120 million infected, ~40 million disfigured); 35 African countries are endemic, with mass drug administration (MDA) coverage of 62% in 2018 and morbidity services lagging. Keast 2014 · Alavi unknown · Deribe 2020
  • Hygiene-based care matters most: acute dermatolymphangioadenitis (DLA) OR 0.32, and self-care halves attacks. An unclean limb doubled the hazard of an acute attack in Tanzania (HR ~2.4), and the rainy season doubled it again. Stocks 2015 · Douglass 2016
  • Doxycycline does not help established filarial lymphoedema: four double-blind LEDoxy trials (India, Mali, Tanzania, Sri Lanka; ~960 patients, 24 months) found no benefit of a 6-week course added to hygiene care; placebo patients improved as much or more — moderate certainty of no benefit (revised 2026-10-09). The ISL 2023 statement that doxycycline may help is not supported for stages 2–3. Krishnasastry 2024 · Coulibaly 2024 · Ngenya 2024 · Yahathugoda 2024
  • Antifilarial drugs (diethylcarbamazine, albendazole, ivermectin) clear microfilariae; debulking in one or two stages is reserved for advanced (grade 3–4) disease. Executive Committee of the International Society of Lymphology 2023
  • Podoconiosis: endemic in 18 African countries (up to 8% prevalence); prevented by footwear. Deribe 2018
  • Not all tropical lymphoedema is filarial: consider podoconiosis, Kaposi sarcoma (HIV) and primary lymphoedema (ask about family history; look at the eyelashes). van Zanten M 2019 · Ebrahim 2009
  • Psychological burden is severe and belongs in care. Person 2008

Lymphatic filariasis

  • Diagnosis: the filarial antigen card test (listed in the SA statement); programme diagnostics shift to rapid antigen mapping once MDA starts. Lymphoscintigraphy is abnormal in 58% of clinically normal opposite legs. Davey 2018 · Molyneux 2009 · Shelley 2006
  • Field measurement: 7-level circumference including the foot tracks volume (r=0.91) but reads low; tissue tonometry. Pani 1995 · Kar 1992
  • Infection: bacteria in 75% of tissue fluid and lymph samples; benzathine penicillin every 2–3 weeks for a year cut recurrent DLA to 16% v 76% (non-randomised, self-selected; the abstract wrongly says "randomly selected") — very low, but consistent with the PATCH trials. Wound myiasis affects 2.5% of swollen legs in Recife; oral ivermectin works. Olszewski 1994 · Olszewski 2005 · Dreyer 2008
  • Doxycycline details: Tanzania's "halted progression" claim comes from a post-hoc analysis; rare serious harm (hepatic encephalopathy in undiagnosed Wilson's disease). Do not add doxycycline.
  • Integrative care (Kerala, India): bandaging, skin care, Ayurveda, yoga and family carers — inflammatory episodes 85% → 9% of limbs and −43% volume in large legs at 3 months (uncontrolled); 14% volume reduction among completers in 2003–06, with 53% not completing; village programmes report large reductions in adherent patients only (28% unaccounted for) — all very low. The AYUSH-SL placebo-controlled trial found both arms improved similarly (about 8% v 7%), with p values that mirror a baseline imbalance; the developer sponsored and analysed it — very low. Narahari 2011 · Narahari 2007 · Narahari 2013 · Narahari 2020 · Mitra 2025
  • Benzopyrone (coumarin) trials in filarial lymphoedema are unreliable. Badger 2004
  • Surgery: a grade IV filarial leg held for 10 years after staged suprafascial excision plus node-venous shunt with CDT (one case); distal LVA after node-venous shunt; genital elephantiasis debulking. Liposuction in filarial legs was "disastrous" (chronic ulcers). Sivaprakasam 2018 · Jamal 1981 · Tripathi 1990 · International Society of Lymphology 2003
  • Overview and programmes: clinical overview; elimination in Tanzania; collaboration between lymphology and filariology; Rwanda had 1 antigen-positive person in 797, so its leg lymphoedema is likely podoconiosis or other causes. Rajasekaran 2017 · Malecela 2009 · Addiss 2007 · Ruberanziza 2009
  • Patients' view (Recife): told the swollen leg was "unavoidable"; acute attacks were the worst burden. Dreyer 2007

Podoconiosis

  • Asymmetric, ascending leg lymphoedema of barefoot farmers on volcanic soils, starting in the foot. Managed with shoes, daily washing, emollient or keratolytic (Whitfield) ointment, compression and elevation; look for venous disease or heart failure when it fails (one report has a P&G co-author). Skin-barrier care underpins management of podoconiosis and filariasis. Fuller 2013 · Davey 2010 · Ryan 2011 · Ryan 2016

Climate, HIV and services

  • Heat and humidity worsen swelling and garment tolerance and trigger filarial attacks (systematic review). Witt 2021
  • Kaposi sarcoma (HIV): compression added to chemotherapy is unproven (Kenyan pilot RCT, very low). Chang 2022
  • Services: Ugandan surgical camps in 2024 treated 376 patients (filariasis, podoconiosis) with no follow-up system and no reported outcomes; low-resource care models; an African surgical perspective with Tanzanian co-authors. Lawin 2025 · World Alliance for Wound and Lymphoedema Care 2009 · Silvestri 2024

Textbook points

  • Hydrocele is the main surgical burden of filariasis (548 000 cases reported to WHO in 2019; up to 83% of its economic cost in India and Africa). Filarial antigen persists for years after cure; colour Doppler "filarial dance" locates live worms. Lymphangiosarcoma complicating filarial lymphoedema is rare and under-reported. Silvestri 2024
  • Indian surgeons report that attacks fall with hygiene, foot care and topical creams rather than with antibiotics or diethylcarbamazine, and advise against pneumatic pumps in filarial legs; filarial lymphoedema reflects lymphatic dilatation and pump failure rather than obstruction, so LVA does poorly. Neligan 2016 · Lee 2018 · Harder 2024

Gaps and warnings

  • The benefit of the hygiene package itself is uncontrolled (very low) but consistent across all sites.
  • Integrative and Ayurvedic programmes report outcomes for completers only, and the one placebo-controlled trial was developer-run.
  • No data on filariasis in South Africa (not endemic; relevant to migrants).

For the therapist

  • Do: teach daily washing, drying, emollient and footwear; treat entry lesions; manage acute attacks promptly.
  • Stop: adding doxycycline for established filarial lymphoedema (moderate certainty of no benefit); pneumatic pumps in filarial legs.
  • Certainty: hygiene packages very low but consistent; doxycycline moderate (no benefit).

For a vascular surgeon in South Africa

  • Filariasis is not endemic in SA, but consider it, and podoconiosis, in migrants from endemic countries; use the antigen card test.
  • Kaposi sarcoma lymphoedema is the more likely local "tropical" cause. Hygiene and footwear are cheap and effective; penicillin prophylaxis for recurrent attacks is reasonable (weak filarial data, consistent with the cellulitis trials); do not add doxycycline.
