Evidence in one minute
- CDT has two phases: an intensive phase of skin care, MLD, exercise and multilayer short-stretch bandaging, daily for 2–4 weeks; then a maintenance phase of garments, self-care, exercise and self-MLD. Every guideline held treats it as the standard, on long experience rather than trials. Executive Committee of the International Society of Lymphology 2023 · Lymphoedema Framework 2006 · Davey 2018
- Compression does the work. No conservative treatment is proven superior to another, and harms are rare. Intensive two-phase therapy has not beaten garments alone in early BCRL (very low; the one adequate RCT was null). Oremus 2012 · Jeffs 2024 · Dayes 2013
- MLD adds nothing to compression and exercise for volume in chronic BCRL: in EFforT-BCRL (194 women with chronic breast cancer-related lymphoedema, double-blind, sham MLD as control), excess volume fell by about 5 percentage points in all three arms; fluoroscopy-guided MLD v placebo MLD −0.2 percentage points (−2.1 to 1.8), and v traditional MLD 0.0 (−2.0 to 2.1). Function, quality of life and erysipelas did not differ either — moderate certainty of no clinically important benefit (revised 2026-10-09; figures corrected 2026-10-31). Use MLD for areas that are hard to compress (head and neck, breast, trunk); its effect on pain and heaviness has not been tested properly. De Vrieze T 2022
- Self-CDT after teaching by a certified therapist matched therapist CDT (−568 v −567 mL to 3 months) — low; relevant to SA staffing. Gultekin 2025
- Exercise is safe: supervised progressive resistance training 2–3 times a week does not worsen BCRL (moderate, ACSM 2019); aerobic exercise appears safe but the evidence is insufficient. The 150 minutes a week of moderate activity quoted by the ISL is general advice for cancer survivors, not a lymphoedema-specific dose. Campbell 2019 · Davies 2026
- Taping: no volume benefit (low); function and comfort gains very low — the review's abstract omits the null main outcome (revised 2026-10-08). Yang 2024
- Drugs: benzopyrones (coumarin, oxerutin, diosmin) give no reliable benefit and can damage the liver (Cochrane); diuretics are of marginal benefit and possibly harmful long term; no diet is proven. Badger 2004 · National Lymphedema Network Medical Advisory Committee 2011
- After surgery, conservative therapy continues for life; there is no evidence-based rehabilitation protocol around microsurgery. Flores 2024 · Doubblestein 2023
The CDT package
- Certainty: the compression component is moderate to high in BCRL; MLD as an add-on is moderate certainty of no important effect; the full CDT package is accepted practice with low-quality comparative evidence.
- Timing and course: most volume reduction occurs in the first 2–3 days; CDT works better when started within a year of onset; lymphoscintigraphy may predict response. Yamamoto 2008 · Michopoulos 2021 · Hwang 2007
- Populations: leg lymphoedema after gynaecological cancer (China); children with lymphoedema praecox. Liu 2021 · Abakay 2023
- Intensive phase alternatives: adjustable wraps are non-inferior to bandaging over 10 days (low, manufacturer-funded); taping instead of bandaging gives a smaller reduction (low); 45–55 mmHg bandages were not better than 20–30 mmHg (21 patients, very low) despite the abstract. Reisshauer 2025 · Pajero Otero V 2022 · Duygu-Yildiz 2023
- Maintenance: a night sleeve after intensive therapy kept excess volume down over 3 months (−16% v +12%) — low, manufacturer-funded. Borman 2026
- Self-care training works: filarial acute attacks fell by 54%, and home exercise with deep breathing reduces cancer-related volume. Douglass 2016
Manual lymphatic drainage
- Physiology: MLD acutely increases lymphatic pumping (ICG velocity +23–28%) and local tissue water falls after one session; it opens limb-root pathways on scintigraphy (fixed sequence), though one session did not change tracer transport in legs. Tan 2011 · Mayrovitz 2008 · Barbieux 2022 · Kafejian-Haddad 2006
- Trials: Cochrane — about 7% of excess volume added to bandaging (2 RCTs, 83 patients; mL outcomes not significant) — low; its "mild BCRL" subgroup came from a data-driven cut-point search (very low; revised 2026-10-08). No extra reduction over bandaging and exercise (Brazil, n=57); MLD roughly equals pneumatic compression after a sleeve. A meta-analysis found no benefit for treatment (SMD −0.09) or prevention (RR 0.61, 0.29–1.26) — low; its two "positive" subgroups use the same data. A review of reviews agrees, and finds exercise helps pain and QoL. Ezzo 2015 · Bergmann 2014 · Johansson 1998 · Liang 2020 · Gilchrist 2024
- Why compression matters more than massage: in obstructive leg lymphoedema the fluid lies deep in the subcutis; hand pressure of 40–>100 mmHg moves it, but pressure falls to zero on release and flow stops or reverses, so compression must follow at once. Neligan 2016
- Keep MLD gentle: forceful massage at 70–100 mmHg damages lymphatic endothelium within minutes. Eliska 1995
- Therapist opinion is moving the same way: less MLD, more compression, exercise and weight loss. Nikolaidis 2013 · Marco 2014
Exercise and other add-ons
- Prevention: pooled RR 0.71 for cancer-related lymphoedema (low), not significant for breast cancer alone (revised 2026-10-09). Plinsinga 2026
- In treatment: supine cycling in bandages gave about 1% more immediate leg volume reduction (low); water v land exercise made no difference (non-randomised); implementation intentions improve adherence. Abe 2021 · Lindquist 2015 · Todd 2007
- No volume effect: myofascial release (3 mL difference; may help shoulder pain); virtual reality v PNF exercise (Egypt) — low to very low. Kim 2023 · Atef 2020
- Low-level laser: fibrotic skin softens and loses water more than with sham — mechanistic only. Mayrovitz 2011
Palliative care
- Half of palliative inpatients have leg oedema; CDT can be adapted; subcutaneous cannula drainage is an option in advanced cancer. Best 2018 · Towers 2010 · Priest 2023
Gaps and warnings
- One CDT meta-analysis found all pooled effects null, with estimates outside their own CIs, yet concludes that CDT works — disregard it. Shamoun 2023
- No double-blind trial of the full CDT package exists.
- Therapist textbooks list contraindications that rest on tradition rather than evidence — decompensated and even compensated cardiac oedema, active cancer, all heat, and blood pressure readings on the at-risk arm. CDT has been given safely with locoregional cancer, and graduated compression is acceptable in compensated heart failure. Zuther 2013 · Wittlinger 2019 · Gloviczki 2024
For the therapist
- Do: prioritise compression, exercise, skin care and self-management teaching; teach self-CDT where therapist time is scarce (equal volume results, low); start resistance exercise early.
- Stop: routine MLD for volume reduction in chronic BCRL (moderate certainty of no added benefit); forceful massage.
- Keep MLD for: areas compression cannot reach (head and neck, breast, trunk, genitals) and for patient comfort, explaining that its benefit for pain and heaviness is untested.
- Certainty: MLD add-on moderate (no benefit, chronic BCRL); self-CDT low; exercise safety moderate.
For a vascular surgeon in South Africa
- Certified therapists need at least 135 hours of training (LAOSA register), and medical aids often fund poorly.
- Where therapist time is scarce, a simplified, compression-led programme with taught self-CDT is realistic and supported by trial data.
